The Cleveland Guardians Vision Plan is administered by Medical Mutual of Ohio and utilizes the EyeMed Network. When you select vision coverage, you and your covered dependents may receive an eye examination once every 12 months. In addition, the plan covers all or part of eyeglasses, frames or contact lenses every 12 months. The Vision Plan uses a broad-based network of providers. Click here to locate an eye doctor in the EyeMed Network (www.eyemed.com). Once you’ve used your Medical Mutual Vision Care Benefit, you can save 20% off a complete pair of prescription eyeglasses with in-network providers.

2. Register using your member ID or the last four digits of your social security number. (You’ll get an email asking to confirm your account.)
3. Finish setting up your new account with your email address and a password.
