The Cleveland Guardians are proud to offer a comprehensive dental program that allows you to access your choice of provider. The plan is administered by Medical Mutual of Ohio and provides comprehensive coverage for preventative, basic, restorative and orthodontic care.
ID Cards
Medical Mutual will distribute ID cards for the dental plan if you elect this as a stand alone benefit but for most members you will utilize the one combined card for Medical, Dental and Vision services. When you go to the dentist, simply provide your SSN to verify coverage.
Log on to Access My Health Plan
- Visit www.medmutual.com
- Select “Member Log-In” area on the top right hand side of the screen
- Click “Register for an Account” to create your login and password
- Register using your Name, Medical Mutual ID Number, Date of Birth
- Create a user name and password
